New vs. Refurbished: What to Look for in a Dental Chair, CT Scanner, and Biosafety Cabinet

For the past six years, I've been the procurement manager at a 30-person dental and orthopedic surgery group. I manage a $620,000 annual equipment budget, I've negotiated with more than 40 vendors, and I track every purchase order in a cost system that has survived three different ERP migrations.

If you're trying to decide between new and refurbished equipment, here's the honest version. Not the brochure version.

Why I stopped comparing sticker prices

When I audited our 2023 spending, the biggest mistake I see isn't buying expensive equipment. It's buying cheap equipment without calculating the full cost of ownership. A $12,000 dental chair that needs $4,000 in repairs over three years is more expensive than an $18,000 chair that works without drama.

That's why this article isn't a list of price tags. It's a comparison between new and refurbished equipment across the three categories I've bought most often: dental chairs, CT scan machines, and biosafety cabinets.

The goal is simple: give you a decision framework, not a sales pitch.

The comparison framework

In every equipment category, I compare five things: upfront cost, hidden installation and commissioning costs, reliability, safety and compliance, and service after the sale. I look at total cost over a three-year horizon because that's usually when the warranty ends and the real costs begin.

What was best practice in 2020 may not apply in 2025. Certified refurbishers have gotten better, and service contracts have become more flexible. But the fundamentals haven't changed: you need documentation, support, and a backup plan.

I have mixed feelings about refurbished equipment. On one hand, the savings can be massive. On the other, I've watched a $7,000 'deal' become a $15,000 problem when the warranty did not cover recalibration. How do I reconcile that? I use three-year total cost. It's not glamorous, but it works.

Dental chairs: new vs. refurbished

Dental chairs are the closest thing to a commodity in this conversation. A solid new chair costs between $18,000 and $35,000. A refurbished unit from a reputable dealer runs $8,000 to $15,000. If you compare only line items, the refurbished chair looks like the obvious choice.

Then you look at the hidden details. The refurbished chair I bought in Q2 2024 didn't include a new foot control, the water bottle bracket didn't fit the model, and the upholstery had a color that didn't match our operatory's design. That's not a clinical disaster, but it added $1,850 in replacement parts and two days of lost operator time.

Here's my conclusion: if you're buying for a high-volume practice and plan to keep the chair for 8-10 years, buy new. If you're buying for a second operatory or a satellite clinic with low usage, a refurbished chair from a certified dealer can be viable.

What to look for in a dental chair (beyond the price tag)

I've installed more dental chairs than I can count, and the details that matter are rarely in the sales brochure. Here's what I check now:

  • Motor duty cycle. A chair rated for continuous duty will outlast one rated for intermittent use, even in a low-volume clinic.
  • Serviceability. Can a local technician get parts without waiting four weeks?
  • Infection control. Smooth surfaces, no upholstery seams facing the patient, and easy replacement panels.
  • Electrical compliance. I look for IEC 60601 compliance. That's the standard that covers basic safety and essential performance.
  • Warranty language. Does labor coverage exist after the first year? That matters more than the headline warranty period.

The short version? A dental chair that looks identical in a photo can perform differently in a clinic. Simple as that.

CT scan machines: new vs. refurbished

Buying a CT scan machine is not like buying a dental chair. A new cone-beam CT scanner can run $150,000 to $300,000. A refurbished unit might be $60,000 to $150,000. The savings are enormous. So I was very tempted to go all-in on refurbished CTs.

In 2021, we bought a refurbished CT scan machine from a large liquidator. The upfront cost was 42% below a comparable new unit. Then the software license fee hit. Then the installation contractor found the gantry needed recalibration. Then the previous owner's imaging protocol didn't match our workflows. Total extra cost: $28,000.

That doesn't mean refurbished CTs are bad. It means the buyer has to verify more than the scanner itself. Here's the exact method I use: confirm the refurbisher has factory-trained service engineers, get the service history in writing, and verify that the software version is still supported by the original manufacturer. I also ask for the FDA 510(k) clearance number for the specific model. A refurbisher who cannot produce that documentation is a non-starter.

For CT, my conclusion is conditional: a certified refurbished unit can make sense if the service contract and software support are identical to what you'd get with a new unit. If they're not, buy new. The gap in total cost is much smaller than the gap in sticker price.

Biosafety cabinets: the one I'd almost never buy used

Biosafety cabinets are the most under-bought piece of equipment in the whole dental and surgical world. People treat them like expensive air filters. They're not. They're infection control devices. A good new Class II A2 biosafety cabinet costs $9,000 to $15,000. A used one might be $4,000 to $7,000.

I bought a refurbished one once. Bad decision. Actually, the decision wasn't bad; the lack of a service contract was the real problem.

The cabinet passed a smoke test. It had current NSF/ANSI 49 certification. It looked flawless. Four months later, the airflow monitor went out of range during an annual validation. The local service company quoted $3,800 to rebalance and recalibrate. The savings disappeared.

Here's the difference between this and the CT conclusion: with a CT scan machine, a certified refurbisher usually has access to factory training and software. With biosafety cabinets, the performance depends heavily on installation, ducting, and room pressure. Those are site-specific. Unless the dealer provides a full performance verification on site and a warranty that includes a second one, I would not buy a used biosafety cabinet. Period.

The Zimmer Biomet verification habit

Now, a different kind of comparison: brand-new products from an authorized channel vs. gray-market products that carry the same brand name.

In our procurement system, Zimmer Biomet shows up in everything from surgical instruments to dental implants. The brand is well known for orthopedic implants, dental implants, instruments, and the ROSA robotic surgery platform. Because it's a brand clinicians trust, it's also a brand that gets counterfeited. Whenever I see “zimmer-biomet” on an invoice line item, I know the brand name won't protect us if the product is fake.

I learned this the hard way. A salesperson offered us a lot of “Zimmer Biomet” dental implant components at a 35% discount. The packaging looked right. The logo looked right. The serial numbers were not.

When I checked the official Zimmer Biomet corporate website, I found that the distributor was not on the authorized list. I then contacted the company's customer verification line, and they confirmed the serial numbers didn't match any recognized production batch. That order never got paid.

If your budget is tight, my advice is more specific. Buy a certified refurbished CT scanner only if the service contract matches a new unit. Buy a refurbished dental chair only from a dealer that includes a full reassembly and a one-year labor warranty. Do not buy a used biosafety cabinet unless you can get a current NSF/ANSI 49 field certification report and a site visit from the seller.

None of this is universal. My experience is based on about 300 equipment orders for a multi-site clinic group with a full-time biomedical technician. If you're a solo dentist with one operatory, your experience might differ. You might not have time to chase down a liquidator or manage a refurbishment project. That's okay. The principle doesn't change: compare total cost, verify safety, and document every decision.

I've only worked with domestic vendors in the U.S., so I can't speak to how these principles apply to international sourcing. But the first step in any market is the same: check the source before you check the price.

That's what procurement looks like when it isn't just buying the cheapest thing on a list. It's buying the thing you can still trust three years later.
